In the space provided below, please indicate the quantity you would like to order. It is important to choose the correct type of certificate (student/faculty/alumni/honorary) as the appropriate designation is added to each blank and imprinted certificate.

Please list all conferees. Include their status (student, faculty or alum), full name, and credentials (faculty or alum - 8 characters max.) List only those requiring a printed certificate

When you click the submit button, your order will be sent. Once received, your order will be assembled and mailed to the address provided along with an invoice for the order including shipping and handling.

Ordering Guidelines

Orders from multiple departments or programs should be aggregated into a single chapter order
Please have a single designated contact person input and receive the order, and initiate payments.
If possible, please pay by PayPal (Credit Card). You may request a PayPal link when you receive an email indicating the order was sent. Please pay the same day you request a link for tracking purposes.
All payments are due upon receipt of the invoice. Payments should be remitted before additional orders are placed.
We can provide W-9’s and associated information if needed. As we are a volunteer organization we are trying to avoid having to create vendor accounts that we have to manage as it adds to the workload significantly.
